Behavior support services is a direct/in-direct service that includes: functional assessment; the development of strategies to support the individual based upon assessment; and the provision of training to individuals, staff, parents and caretakers. Services must be required to meet the current needs of the individual, as documented and authorized in the ISP.
Behavioral Support services are provided in the natural environment by a qualified individual. Services will be utilized in the client’s home and community settings in order to observe behaviors, collect data, develop a behavior support plan and provide support to the client as they make the behavior changes necessary to achieve their goals. Behavioral Support staff will collaborate with clients and their natural supports to ensure that proper training is provided, that the goals being addressed are socially significant, and to provide ongoing monitoring and implementation of the plan using positive practices. The plan will be revised annually or as needed, based upon display of behaviors and progress made.
In-Home and Community Services is a direct service provided in home and community settings to assist individuals in acquiring, maintaining and improving the skills necessary to live in the community, to live more independently, and to participate meaningfully in community life. Services consist of assistance, support and guidance in general areas such as (but not limited to) self-care, health maintenance, decision making and home management.
In-Home and Community Support needs will be determined through observation, assessment, data collection and collaboration with the client and their natural supports to determine areas of greatest need that may be targeted for skill development. IHC staff will provide support to the client as they move through their daily routine and provide guidance to develop skills necessary to becoming more independent. The services will be provided in the client’s natural settings for those activities, home or community depending on the activity.
Community Participation Supports is a direct service that supports clients to participate in integrated community settings, in activities that involve persons without disabilities who are not paid or unpaid caregivers. The primary goal of CP staff is to increase the client’s opportunity to build connections within their local community and include (but are not limited to) the following supports for:
· skill development;
· Participation in community activities, groups, associations, or clubs to develop social networks with community organizations and clubs;
· Identification of and participation in activities that provide purpose and responsibility;
Community participation goals will be determined through observation, data collection and collaboration with the client and their natural supports. CP Staff will provide individualized support to the client in those community environments where they have expressed a desire to develop their skills and increase participation. CP Staff will also work with the client to increase the scope of activities that they participate in, ensuring they also include activities that provide purpose and responsibility.
Companion services are direct services provided to individuals who live in private homes but may also take place in the community and/or work setting. Companion services provide supervision or assistance that is designed to ensure the participant’s health, safety and welfare or to perform activities of daily living for the participant so that they participate in home and community life in a meaningful way.
Respite services provide relief to caregivers in their absence within the individual’s private home or outside of the home in a setting acceptable to the individual of their family/guardian.
